Yeah I have the exact same problem, I think it's a memory recall issue. I really struggle with open questions in general. "What is your favorite X?" I definitely have a favorite but I can't recall it at the moment so either I'll blank out or spout the first thing I think about even if I don't like it that much. That's the reason I have txt files on my desktop with lists of my top 10 favorite songs, games, anime, etc "What did you do last week/this weekend?" My therapist asked me this and I was stuck for 30 seconds cause my mind was blank. I would say it's just hard to recall stuff that's the main problem. Also gotta prioritize the important stuff or events that differ from the norm. Memories might not have saved properly too. Generally it's a much more open questions than you'd expect. "What's your name?" I used to take like 5 seconds to answer that. First because I'm blanking out for a few seconds but then also cause I'm trying to figure out if they want my first name or last name or both. Anxiety has a role but it's not just anxiety cause even now thinking of these questions in my head like I have no idea what my favorite video game is or what I did last week or what I ate last night idk like it would take me some time and retracing my steps to recall

I can relate to the answering questions part and that is most likely issues with “working memory” - the brain has trouble with short term memory and recall on the fly.
