Like I can’t really explain why, but I’ll feel the compulsion to take over-the-counter sleep meds until I feel more loopy and relaxed than normal, or to (getting to the point of your post) drink with the intention of numbing or dulling or something. There’s just a part of me that’s always looking for a chance to get relief from whatever it is that’s malfunctioning inside me. When I hit my correct dosage of meds, however, this compulsion is just like; not even in my brain. I just feel a sense of both balance and presence of mind that just feels like what my brain is actually supposed to be doing. I’ll then start to feel the *opposite*, which is what I imagine a normal person often feels: I don’t WAN’T to take enough of any certain substance that it may start to affect my thinking, feeling, and functioning. I literally transform from someone who doubles the recommended dose of any and all medicines, because *what the hell, I wanna feel something* to someone that literally turned down any type of pain medicine at the ER after slicing my entire forehead open in a bicycle accident, because I had a lot going on that week and wanted to stay clear-headed for. So I guess a TL:DR of my thoughts might be: if the root of whatever it is that causes you to abuse substances is located somewhere in your ADHD, as I suspect mine is, then proper medication can absolutely help it like you’ve described here.
