If you normally attend doctors appointments with your mom, you could bring it up during an appointment. You could also call the doctors office and leave a message asking that she be evaluated.  Realize that it is not at all simple to take over a family member's affairs and it sounds like she is likely to push back, so don't gang up on her! Small conversations over time are the best tactic in my experience.
